Contribution Differential for Opt-Out by or Removal of Covered Individuals from the Wellness Program |

Employees will pay a $600.00 per year contribution differential for each covered individual who opts out of the Wellness Program, prorated to a per pay period basis, or who is removed from the Wellness Program for failure to participate in wellness activities.
